Chapter 2228: Chapter 1453: Invisible Hand (2)
Yang Ping handed the paper back to Jiang Jitong: "Regarding the patent issue, follow Tang Shun’s arrangements. Continue with the experiments; I need to see the results from the large animal experiments. No matter how well the small animal tests go, things can still go wrong with the larger animals."
"Understood!" Jiang Jitong nodded, clutching the documents as he walked briskly away.
Yang Ping watched his back, recalling how Jiang Jitong appeared when he first came to Sanbo. He was a PhD candidate who had passed the exams from another institution. During the interview, he was so nervous that he could barely speak, but when it came to professional discussions, he transformed into a different person—clear, systematic, and quick-thinking.
In retrospect, Tang Shun’s judgment was accurate; this person is capable of great responsibilities.
At ten in the morning, Dean Xia arrived as scheduled.
This time, he didn’t bring blueprints but carried a USB drive instead. Upon entering the office, he directly inserted the USB into Yang Ping’s laptop and opened a 3D model.
Yang Ping’s office has a laptop specifically for connecting external devices, as it’s isolated and not online, so even if the USB carries any virus, there’s no concern.
Actually, Yang Ping doesn’t really understand these professional aspects of laboratory design; Dean Xia’s visit is merely procedural. Yang Ping glanced at the model, expressed no objections, and understanding Yang Ping’s busy schedule, Dean Xia left quickly after completing the formalities.
Yang Ping returned to his desk, sat for a while, then turned on his laptop to start handling today’s emails.
One email caught his attention; it was sent from Harvard Medical School, signed by Professor Green from the Neurosurgery department. In the email, Professor Green mentioned that their team has been conducting research on brain glioma immunotherapy and encountered a challenge, seeking Yang Ping’s advice. He attached the research data, the tone very sincere.
Yang Ping carefully examined the data; Professor Green’s team discovered a new tumor immune escape mechanism but couldn’t explain how this mechanism gets activated. Yang Ping thought it over, mentally reviewing the core framework of the three-dimensional gene orientation theory, then started to compose his reply.
He wrote slowly, each sentence carefully considered; this was not just courteous academic exchange but genuinely tackling the issue. His response needed to withstand scrutiny, with no room for ambiguity.
After writing for forty minutes, he finally hit the send button.
The email’s last sentence read: "Above is my preliminary analysis for your reference. If further discussion is needed, we can arrange a video conference."
Putting down the mouse, Yang Ping rubbed his wrist; after typing for a long time, his fingers felt a bit sore.
After lunch and a short rest, at two in the afternoon, Yang Ping appeared promptly in the Neurosurgery Meeting Room.
Xu Zhiliang was already waiting there, with a 3D reconstruction map spread out in front of him. Several backbone doctors from the Neurosurgery department as well as the heads of the Anesthesiology and Critical Care departments were seated beside him.
"Professor, the preoperative preparations... are... nearly... complete." Xu Zhiliang pointed at the 3D reconstruction map, "This is... the latest... MRI, edema... has significantly... subsided. The adhesions... around... the lesion are... lighter than... expected."
Yang Ping took the map, examining it carefully. The lesion at the dorsal side of the medulla, after two weeks of conservative treatment, had reduced significantly in size, with the surrounding low signal ring becoming clearer. This indicated that the timing for surgery was ripe.
"What about the plan for intraoperative electrophysiological monitoring?" Yang Ping asked.
"I sent... you... the... WhatsApp details, including the illness and treatment plan introduction..."
Yang Ping checked WhatsApp, his introduction content was very detailed, from the placement positions for monitoring electrodes to setting alarm thresholds and linking various abnormal waveforms to clinical significance; every detail was thoroughly explained.
After reviewing, Yang Ping nodded: "The plan is fine, execution needs to be precise. Particularly the monitoring of the hypoglossal nerve and vagus nerve, these two nerves are the most susceptible to damage in dorsal medulla surgeries. Damage to the hypoglossal nerve can cause tongue deviation and affect speech and swallowing; damage to the vagus nerve might lead to hoarseness, coughing, or in severe cases require tracheotomy."
"Understood." Xu Zhiliang drew a heavy circle in his notebook.
"What about Anesthesiology?" Yang Ping turned towards Old Zhou from the Anesthesiology department.
Old Zhou cleared his throat: "We have prepared two plans; the preference is intraoperative awakening. We’ve assessed the patient; they possess good psychological resilience and a high level of education, so cooperation should be good. If any issues arise during the intraoperative awakening, we will immediately switch to general anesthesia with electrophysiological monitoring."
"How do you ensure airway management during awakening?" Yang Ping asked, considering these fundamental issues to be very important.
"We won’t use tracheal intubation but will utilize laryngeal mask ventilation." Old Zhou said, "The laryngeal mask irritates the airway less than tracheal intubation, making it easier for the patient to tolerate during awakening. We’re also using a new type of laryngeal mask with a suction channel to prevent excessive secretions from causing aspiration."
Yang Ping pondered: "Is the seal of the laryngeal mask sufficient? The surgical position is prone, with the patient’s head fixed; if the laryngeal mask shifts, repositioning would be very challenging."
"We’ve conducted simulation exercises; in the prone position, the seal of the laryngeal mask meets the requirements." Old Zhou said, "We’ve also prepared modified fixation to ensure the laryngeal mask doesn’t leak, and have developed emergency plans. If significant laryngeal mask leakage occurs during surgery, we will immediately switch to tracheal intubation, pause awakening, and deepen anesthesia."
Yang Ping asked no further; Old Zhou is a veteran in Anesthesiology, rich in experience, considering matters more meticulously than him.
"What about Critical Care?" Yang Ping queried the ICU head.
The specialized cranial ICU Director Chen opened his laptop, displaying a detailed post-surgery management flowchart on the projection screen. From the first minute the patient returns to the ICU to the seventh day post-surgery, every time point and corresponding actions are detailed clearly.
"Post-surgery concerns are primarily medulla edema and respiratory center inhibition." Director Chen said, "That’s why we’ve prepared a comprehensive approach involving dehydration, steroids, and respiratory support. If post-surgery there’s any abnormality in breathing rhythm, immediately perform a head CT scan to rule out hemorrhage or edema; if there’s no occupying lesion, continue respiratory support and wait for functional recovery."
Yang Ping gazed at the flowchart, suddenly feeling a bit emotional.
Back when he first arrived at Sanbo, Neurosurgery’s post-surgery management was quite rough. Patients transitioned from the Operating Room to ICU, mostly left to "handle as seen fit." Complications were addressed reactively. But now, every step has a plan, every possibility has a strategy, and there’s a specialized cranial ICU—progress has been rapid indeed.
This isn’t just about one individual’s impressive skills; it represents the enhancement of the entire system’s capability.
"Alright!" Yang Ping stood up, "Tomorrow morning at seven-thirty, the patient enters the Operating Room. Surgery commences promptly at eight. Everyone, double-check your preparations; any oversight can be fatal."
Everyone stood up, packed their items, and left. Xu Zhiliang was the last one to leave. At the door, he turned back: "Professor, tomorrow... will you..."
"I’ll be there on time," Yang Ping assured him.
Leaving the Neurosurgery department, Yang Ping didn’t return to the Institute, but instead walked alone around the hospital’s garden path, then sat in the garden enjoying the breeze; he likes this.
His phone buzzed, he took it out and saw a message from Xiao Su: "What would you like to eat tonight?"
Yang Ping thought for a moment, then typed: "Whatever you cook, I’ll eat. As long as you make it, it’s the best cuisine."
"Then I’ll make my specialty—tomato scrambled eggs."
"Great!"
Reading the message, Yang Ping’s lips curved into a smile; tomato scrambled eggs, that’s Xiao Su’s signature dish. She once mentioned that when she was not yet ten, the first dish she learned to make was tomato scrambled eggs.
He stood up, patted the dust off his pants, and headed towards the Institute.
