Chapter 2229: Chapter 1454: That Shouldn’t Be Right
In the morning, Yang Ping did not go directly to the Operating Room, but first went to the Neurosurgery Ward.
The patient with cavernous malformation of the medulla is surnamed Gong, forty-seven years old, a middle school language teacher, has taught for twenty-five years, and has led eleven graduating classes of senior high school. Yang Ping had memorized this information last night, but he still wanted to see the Patient in person and hear his voice before Surgery.
The lights in the Ward were already on, Teacher Gong was leaning against the bed, his wife sat by the bedside.
Yang Ping nodded and walked to the bedside. Teacher Gong’s complexion was a bit better than the photo seen two weeks ago, his face was not so pale anymore, and his eyes were much clearer. He was wearing a blue and white striped hospital gown, his hair was shaved off, exposing a smooth Head with a few faint spots, marks left by a tan.
"Teacher Gong, how are you feeling?" Yang Ping asked.
"Alright." Teacher Gong’s voice was light but steady, "My Head is still a bit dizzy, walking is unstable, but much better than when I first arrived."
"How about eating, any choking?"
"No choking, eating is fine, drinking water is also okay." Teacher Gong paused, "just sometimes I feel tightness in the throat, like something is stuck there."
Yang Ping felt a tightening in his heart, throat tightness could be a sign of impaired medullary function, or perhaps an edema around the lesion pressing against the vagus nerve. Regardless of the situation, it presented a reason that Surgery couldn’t be delayed.
"Surgery is today." Yang Ping said, "I need to tell you a few things, listen carefully."
Teacher Gong nodded, his wife tightly held her husband’s hand.
"Firstly, during the Surgery there will be a period where you are awake. You will hear us talking, might feel your Head being opened, but there will be no pain. Don’t be nervous, just follow the instructions of the Anesthesiologist. Take deep breaths, wiggle your toes, say a few words, cooperate with us on these checks."
"Secondly, during the Surgery some conditions might occur, for example, your hand or foot suddenly can’t move, or speech suddenly becomes unclear. If that happens, don’t panic, it might be because we’re close to an important Nerve, and we’ll stop the operation immediately, wait for you to recover then continue."
"Thirdly, and most crucial." Yang Ping looked into Teacher Gong’s eyes, "No matter what happens, don’t move erratically. If you move, our operation might deviate, causing irreversible damage. So no matter how uncomfortable, bear with it, stay calm."
Teacher Gong was silent for a moment, then nodded.
"Professor Yang, I have one more question."
"Go ahead."
"If... I mean if... an accident occurs during the Surgery, will you focus on saving my life, or preserving my function?"
Yang Ping looked at this forty-seven-year-old middle-aged man, who taught for twenty-five years, led eleven graduating classes of senior high school, persuaded a Student attempting to jump from a building. Asking this question shows he has seriously thought about the worst outcome, shows he’s mentally prepared, and shows he’s genuinely a rational person.
Yang Ping said, "If such a situation arises, theoretically, we prioritize saving your life, but given my technical expertise, both can actually be preserved."
"Great, thank you." Teacher Gong said reassuringly.
Yang Ping exited the Ward, stood in the hallway for a few seconds. A Nurse wheeled a treatment trolley past, its wheels made a faint rumbling sound on the tiled floor. He took a deep breath and headed towards the Operating Room.
The profession of Doctor truly carries a divine nature, sometimes beyond having the skills, one must also have a conscience.
At eight o’clock, everyone was already in place in the Operating Room.
Xu Zhiliang stood by the Operating Table, finalizing details with the Anesthesiologist. His assistant, a young Doctor in his thirties, was helping to position the Patient, securing the head in a head holder, ensuring it wouldn’t shift in the prone position. Nurses were counting the instruments, reporting numbers one by one, their voices crisp and efficient.
Yang Ping walked to the scrub basin, began washing hands. Xu Zhiliang followed over and stood beside him. The two of them scrubbed hands side by side, neither speaking, only the sound of water flowing and brushes scrubbing against skin.
After scrubbing hands, donned the Surgical Gown, gloves on. Yang Ping walked to the Operating Table, stood at the First Assistant position, Xu Zhiliang stood in the Chief Surgeon position.
The Surgery commenced in silence.
Xu Zhiliang picked up the Surgical Knife, made a median longitudinal Incision in the posterior neck of the Patient. Skin, subcutaneous tissue, fascia, were severed layer by layer. His moves were slow, each cut precisely measured. This was his style, unhurried, unflustered, steady as a rock.
Yang Ping watched alongside, internally praising him.
Muscle was gradually separated, exposing the occipital foramen and the posterior arch of the atlas. Xu Zhiliang took the drill, began the posterior fossa craniotomy. The drill’s sound echoed in the Operating Room, interwoven with the sound of the monitoring device.
The bone flap was carefully removed, exposing the dura mater, white, smooth, pulsating slightly. Xu Zhiliang used a micro Scissor to cut open the dura mater, cerebrospinal fluid seeped out and was suctioned away by the Suction Device.
"Brain pressure is not high." Xu Zhiliang said.
Yang Ping nodded, low brain pressure was a good sign, indicating that two weeks of conservative treatment had produced the expected effect.
The Microscope was brought over, Xu Zhiliang sat down, eyes against the eyepiece. His assistant also sat on the other side, Yang Ping stood by, observing the surgical field through the auxiliary eyepiece of the Microscope.
The cerebellar hemisphere was gently separated, exposing the bottom wall of the fourth ventricle. It was a thin, paper-like structure, beneath which lay the dorsal side of the medulla. Through this thin wall, the lesion could be seen underneath, a mass of dark red, mulberry-like malformed Blood Vessels.
